TELEHEALTH CONSENT
RestartRX Management LLC ("RestartRX," "we," "us," or "our") is a technology platform that connects patients with independently licensed healthcare providers. RestartRX does not provide medical care, diagnosis, or treatment. All medical services are provided by independent healthcare providers.
By using the RestartRX platform and proceeding with a consultation, you acknowledge and agree to the following:
1. WHAT IS TELEHEALTH
Telehealth involves the delivery of healthcare services using electronic communications, such as:
- Online medical intake forms
- Secure messaging
- Audio, video, or asynchronous communication
- Transmission of medical records, images, and other health data
These methods allow providers to evaluate, diagnose, and treat patients without an in-person visit.
2. POTENTIAL BENEFITS
Telehealth may offer the following benefits:
- More convenient access to care
- Reduced travel and wait times
- Increased access to providers
- More flexible scheduling
3. POTENTIAL RISKS AND LIMITATIONS
You understand that telehealth has limitations, including:
- The provider may not be able to perform a full physical examination
- Information submitted may be incomplete or inaccurate
- Technical issues (e.g., connectivity, system failures) may interrupt care
- There may be delays in evaluation or treatment
- Telehealth may not be appropriate for all conditions
You understand that a provider may determine that telehealth is not appropriate for your situation and may recommend in-person care.
You acknowledge that the inability to perform an in-person physical examination may limit a provider's ability to fully assess your condition.
4. NO GUARANTEE OF TREATMENT OR PRESCRIPTION
You understand that:
- Not all patients qualify for treatment
- A prescription is not guaranteed
- All treatment decisions are made at the sole discretion of the licensed healthcare provider
5. PROVIDER RELATIONSHIP
You understand that:
- Your healthcare services are provided by independently licensed providers
- RestartRX is not a healthcare provider
- Use of the platform does not replace your relationship with a primary care provider
You are encouraged to seek care from your primary care provider or a specialist when appropriate.
6. PRIVACY AND SECURITY
Your medical and personal information will be collected, used, and shared as described in our Privacy Policy and Consumer Health Data Privacy Policy.
While reasonable safeguards are in place, you understand that:
- Electronic communications may carry some risk
- Absolute security cannot be guaranteed
7. EMERGENCY SITUATIONS
Telehealth services are not appropriate for emergencies. If you are experiencing a medical emergency, call 911 or seek immediate in-person care at the nearest emergency department or urgent care facility.
If you are experiencing a mental health crisis or contemplating suicide, call 911 or contact the 988 Suicide & Crisis Lifeline by calling or texting 988.
Telehealth sessions are not routinely recorded by RestartRX. Providers may have their own policies regarding documentation.
8. PATIENT RESPONSIBILITIES
You agree to:
- Provide accurate and complete information
- Update your information as needed
- Follow provider recommendations
- Seek in-person care when advised
Providing inaccurate or incomplete information may impact your care.
9. RIGHT TO WITHDRAW CONSENT
You may withdraw your consent to telehealth services at any time by discontinuing use of the platform and notifying us at support@restartrx.com.
Withdrawal of consent will not affect care already provided but may limit your ability to continue using the services.
10. PHARMACY AND PRESCRIPTION FULFILLMENT
If a prescription is issued:
- You may request that it be sent to a pharmacy of your choice
- Prescriptions are filled by licensed third-party pharmacies
- RestartRX does not dispense medications

11. PATIENT RIGHTS AND COMPLAINTS
If you have concerns about the care you receive through our platform, you may file a complaint with the medical board in your state of residence.

California patients: Physicians and midwives are licensed and regulated by the Medical Board of California. To confirm a license or file a complaint, visit www.mbc.ca.gov or call (800) 633-2322.

12. ACKNOWLEDGMENT AND CONSENT
By using the RestartRX platform, completing a medical intake, or proceeding with a consultation, you acknowledge that:
- You have read and understand this Telehealth Consent
- You understand the risks and benefits of telehealth
- You voluntarily consent to receive healthcare services via telehealth
